You must submit one of the following evidence of U.S. Citizenship:
- U.S. State Certified Birth Certificate
- 16 and 17 year olds must have Certified Birth Certificate and at least one parent
- Fully-valid, undamaged U.S. passport (may be expired)
- Certificate of Naturalization

- Passport Card application fee: $30.00 Check or money order is required
- Expedited service costs an additional $60.00 for application fee
- Execution (Acceptance) Fee: $35.00 cash/card/check.
