As “Disability Access Points” or “DAPs” we help people with disabilities, their families, and their caregivers find the services and support they need—both short-term and long-term. We help give people information, provide referrals and plans, and check in to make sure Iowans are getting the right support to live a healthy, independent life.
The Western Iowa Services Collaborative (WISC) serves as the Disability Access Point for Districts 1 and 4 serving the counties of Adair, Adams, Audubon, Buena Vista, Carroll, Cass, Cherokee, Crawford, Fremont, Guthrie, Harrison, Ida, Lyon, Mills, Monona, Montgomery, O'Brien, Osceola, Page, Plymouth, Pottawattamie, Ringgold, Shelby, Taylor, Sioux, Union, and Woodbury.
District 1 Serves the following counties: Buena Vista, Carroll, Crawford, Cherokee, Harrison, Ida, Lyon, Monona, O'Brien, Osceola, Plymouth, Sioux, Woodbury.

Service Coordination for Non-Medicaid LTSS : This is a process where someone helps the person get and manage their LTSS. The person works with a coordinator to create a care plan, set goals, and choose services to meet those goals.
